Whistleblowing

Whistleblowing Form

PLEASE READ THE FOLLOWING INFORMATION BEFORE CONTINUING WITH YOUR SUBMISSION

Confidential Disclosure

If you’ve decided to provide information confidentially we’ll not ask you to prove anything and will treat the matter in confidence.

Your concerns will be recorded and it is likely that we’ll make internal enquiries first, but it may be necessary to carry out an investigation at a later stage. You will not need to be directly involved with the investigation.

As far as possible, we’ll keep you informed of the decisions we take and the outcome of any enquiries and investigations we carry out.  However, we’ll not be able to inform you of any matters that would infringe our duty of confidentiality to others.  We’ll always confirm to you when the investigation is completed.

Please be aware that there may be occasions when we’re unable to resolve a concern without revealing a whistleblowers identity - for example where their personal evidence is necessary if the concern were to proceed to court.  When this happens, we will discuss with you how to best proceed.

If following discussions with the Society you’d still prefer to keep your identity confidential, we retain the right to pursue the matter further but will respect your decision of privacy.  However, please be aware, depending on the circumstances that this decision may ultimately prevent the Society from being able to proceed further.

Please note that if as a result of our investigation we find that the disclosure was malicious or knowingly false, nothing prevents the Society from taking action against you.

Anonymous Disclosure

By making a disclosure anonymously, a proper investigation may be more difficult or impossible if we can’t obtain further information from you.  It’s also more difficult to establish whether any allegations are credible.

You’ll also not be able to receive feedback.

You should also understand that making a disclosure anonymously means it can be more difficult for you to qualify for protection as a whistleblower. This is because there would be no documentary evidence linking you to the disclosure for an employment tribunal to consider.

Previous step

If you want to disclose anonymously, please leave question 1 blank and do not sign the form at the end.

Optional – do not sign if you are reporting anonymously

By signing this form, you confirm that you are making this disclosure in line with our whistleblowing policy and understand that this matter will be dealt with under this procedure.
